If you do not have a gooseneck trailer and that type of hitch then you do not need to even get that set up. Just get a good fifthwheel hitch and you're good to go.
If you have a gooseneck trailer and want to get a fifthwheel then they make adapters that go on the king pin box, or replacement boxes that are gooseneck.

I have had 2 different 5th wheel hitches and now a gooseneck.
On my 2002 I had a Pullrite Super Glide. It was a great hitch. On the 2012 I had a B&W OEM Companion (fits in the factory 5th wheel prep). It was also a great hitch. When I noticed rust on my pin box on my 5th wheel I decided to change to a Reese Goosebox. This allows me to use a gooseneck ball for my 5th wheel and it is backed by Lippert.
That said I have had both a 5th wheel hitch and Gooseneck on the same truck with the same trailer. I haven't noticed any difference as far as stability. I did notice less chucking with the gooseneck, but I think that is due to the air bag/shock that are in the the pin box now.
I love the gooseneck option. I can have an empty bed in 2 seconds and my hitch connection stores under my seat.

B&W does make a nice turn over ball gooseneck hitch, I will have one in my next truck or if I replace the box on this one. They also make the companion hitch, and a new sliding companion hitch. Really cool hitches.

I'm struggling with this question myself. My last two trucks had B&W turnovers in them. The truck I just traded for has a Curt Q24 5th wheel in it. I sometimes pull a bit more than 24000, but the advantage I see is the ease of hookup. I pull a 40' PJ flat with a hyd tail, and it would be a lot easier to back into the 5th wheel than to crank the trailer up or down to hook to the gooseneck ball. I've heard opinions both ways, and I need to make a decision. Right now, the two don't match!
