What do you mean don't give it gas? I thought you had to tap the pedal before the truck is on to start the high idle. Maybe I misinterpreted.

You /have/ to tap the pedal, and depending on how worn out the engine is, half-throttle it to start it... The visor instuction sheet suggests that you floor it when it's cold out to start it, but I'm not sure about that as it retards the timing somewhat.
Oh yes, it's what you need, they didn't change the part number from year to year, only when there was a change in design.

I've been able to just tap it and go myself.

No. Turn the key on and while your glow plugs are heating, depress the accelerator about half way down and let it back up. That will set the fast idle.
