I have a 1946 1/2 ton with rebuilt engine and tranny. The rebuild was in the late 90,a. If I do not hold it in second gear, it will pop out sometimes and make a noise that scares the bejesus out on you. I obviously will need to pull the transmission. What causes this and should I consider rebuilding the transmission again?
Three or four speed, if 3, it could be a bad synco, or the syno teeth on the gear are worn, or too much back and forth movement in the main shaft. If 4 spd, may be 2nd gear is badly worn, or a shifter fork is bent.

Shifter detents, input shaft bearing, output shaft bearing could be worth checking. I'd also look closely at the needle bearing between the input shaft and main shaft; they are often overlooked.
Check your main shaft end play. There is a spec as I recall. If you replace the bearings make sure the retainer rings are correct width and fit the grooves correctly, should be tight. I learned the hard way. The ones that come with the bearings may not be correct, and allow the main shaft assembly to move back and forth, which can cause it to pop out of gear!!!!
