What usually fails is the foam core, it deteriorates, letting the thin cloth face layer float free.
They used to have a cardboard backing, but years ago switched to a plastic shell.
What I've done - Pulled out enough side trim to drop the headliner with backing. Put face-up on a smooth surface, tore off cloth, and used a wire brush to remove all the dead foam residue. Sweep up and vacuum well.
Went with wife to a fabric store, they often have headliner material. It is a thin cloth bonded to a foam rubber core. Also bought enough 3M FoamFast 74 spray adhesive. Critical to use an adhesive that is compatible with the foam backing, so it doesn't dissolve it and make a horrible mess.
Cut fabric to be larger than the backing. Place on backing, pressing down where needed. Then carefully peel back 1/2 of it, and fold it over the top of the rest of the fabric. Spray adhesive as per directions how much and where. DO NOT get ANY adhesive onto the face side of the fabric, it WILL NOT come off! With 1/2 sprayed, carefully smooth fabric from fold onto the backing, no big air bubbles or bunching, where it touches, it stays. Make first time perfect!
With 1/2 done, then fold up the loose side over the completed side, and repeat procedure.
Spray the adhesive somewhere where overspray won't hurt, or lay out newspaper around the edges. Once you start spraying, there is limited working time before adhesive sets up too much, so when you spray you want to spray and go, and not have to do precision spraying near an edge worrying about overspray.
