There are a couple possibilities. There are two bolts on the steering column that hold the shift tube to the column. If they get loose or fall out this can happen. To check them lay under the driver's side dash looking up the column. The bolts have Torx heads. If they are missing or loose repair them as needed.
If that's fine the next thing to check is to see if a rock got lodged in the linkage. I've seen it happen on the trans where the shift cable attaches on the driver's side of the trans.
