yeah it's hard the first time you do it, but every time in the future will be much easier. You have to use a combination of extensions, might even need to borrow some from a friend to make the right length. It has to be pretty long, but not too long or it will hit something else, there's a small range that works. I also use a u-joint at the ratchet end.
It is (or should be) the same size bolt. I have a combination of 3/8" extensions that puts the ratchet just at the end of the starter for excellent travel. She can be a bear the first time, but you'll get it figured out. Try getting a Sidwewinder turbo off while it is still in the truck!!!!!!!!!!
