Me too! I'm about to get a new windshield and wanted the low-down on the SUPER DUTY logo'd one like I have on it now. (Report back poster award winner strikes again!)
FWIW, I had my windshield in my 1999 replaced last fall and Safelite told me they couldn't get it with the logo or acoustic glass. I'd hate to find out they lied to me.
My glass was not damaged but the windshield was leaking. Glass companies will not R&R your windshield (my original wasn't in great condition to begin with) so I paid out of pocket for the windshield on the 03 about 3 years ago. I had a local glass company do the work at home. They asked me if I want OE glass and if so, do I want the Superduty logo. I said sure. The glass is true FoMoCo and looks great.
One surprising thing was that the urethane was entirely dried up. The windshield came out with a strong push from the inside, only held on by a 12inch stretch of glue on the very bottom. I can't believe it didn't simply fall out. Shock number 2 was that my windshield frame was rust free!
