Perhaps the inspection plate is hitting the flex plate or the heads of the torque converter bolts? Just a guess.

The inspection cover wasn't dented in any where was it? Look on the inside of it and see if there are any marks from something rubbing it.

their was 2 grove marks on the bottom of plate iv'e used the truck twice and no issues, of course running the truck with out the inspection plate, im thinking the plate got bent or loose and was vibrating and was hitting the fly gear

Run it without the inspection cover for a couple of days to confirm the noise is gone.
