That's the keyword there........caught. Some get caught but many do not. There are stations that will check lights, exhaust, slap a sticker on it, and call it a day. It happens more than most people think or want to believe it does.
On our end of the state (SW Va) you can find shops that'll pass just about anything BUT you have to get to know them 1st so they trust you're not a "Trooper" in disguise. The OP sounds like he's new to the state so, he may not be able to find a "friendly" shop as easily as life long res like myself. I've had some iffy vehicles passed but with my word that'll fix'em in short order. One adv we have is no smog test out here although the epa keeps threatening to impose it on the NRV/Roanoke area.
True truck tags are exempt from the "official state insp" however by va motor vehicle code you as the "operator" are to maintain & inspect the vehicle to va state req. Ask how I know this - one of my drivers got pulled over in albermarle co by a trooper and rec'd a 175 dol ticket for not having op brake lights and for not having a working seat belt. (Retractor was stuck). If you have commercial plates or truck is registered for business use you are req'd to perform your own inspections on 90 day intervals. We run a mix of OTR, commercial and truck plates across our farm vehicles and I prefer to stay on the right side of the law - doesn't help when a trooper lives in your "back pocket" either.
I drive for ups and all our trucks get annual va inspections and daily "driver vehicle inspection" which is lights, horn, wipers, tires, wheels/lug nuts, brakes, parking brake clutch, body integrity etc.
On my private vehicle with truck tags i also have the state safety inspection but emissions are exempted.
The mechanics do the 90day insp - its actually a DOT requirement but it only applies to tractor trailers running interstate which ours do. So the UPS delivery trucks & our non otr trucks just get an annual like the 4 wheelers do. We did get audited by the state (motor vehicle insp officer - the ones that you insp big rigs along the side of the road and at weigh stations) once on maint records for our OTR's but it wasn't a big deal - they just wanted to see evidence that the inspections had been done and that any issues found had been addressed
