Sounds like your #6 injector needs replacing. Is it cranky running/driving until it gets warmed up?

The Fram filter is fine (in this case). Absolutely NO NEED to change it early.

I noticed you have two threads going on the same thing and I'll try to get them merged together.

FWIW--the Fram filter is fine. Don't waste your time changing it. IMHO it sounds like you're suffering from some pretty serious injector stiction. You might also want to do the FICM diagnostic procedure. At 250K miles, like Cheezit said, you could have several issues causing a contribution balance code.

I just noticed (duh) that the glow plug symbol isn't coming on on the dash. What is the procedure to check them? Is that info available online anywhere?

The best way is to code scan the truck. It will tell you if you have some kind of fault in the GPCM. It may take something more advanced to diagnose faults in the Hybrid Electric Cluster (dash).
