If the sleeve had been previously replaced and you now see coolant spit out the cylinder, then that sleeve was not re-installed propely and needs to be done again.
If that sleeve was not previously replaced, I don't think it's your problem.

Injector CUPS not cylinder SLEEVES. Just making sure we are all on the same page. He replaced all the injector cups.
Would a coolant system pressure test show the source of the leak? I would tend to thing that would be an easy way to verify the source before going to the cost and trouble or replacing that new cup.
