Injector test with IDS........
You can get a infrared temp gun and check each exhaust pipe at the engine. The one that is way hot or way cold is a stuck open or cracked injector. Even if you could shut that injector off the cylinder would still smoke. I have seen this on 3 different trucks. One guy continued to drive it and melted the piston.

I had the exact same knock several years ago in my 7.3 that's why I suggested it might be a faulty injector.
