My buddy has a beautiful 2005 F-350 Super Crew SRW with the 6.0 Powerstroke and just under 100,000 miles on it. Coming home from the Carolinas this past weekend he started blowing smoke & had a noise......this is what it looks like.
I am no expert but i don't think its a rod knock. Maybe injectors they can make some funny noises , but all that white smoke could be EGR cooler blew and maybe dumping water in intake , then you can guess where it goes next. JMO
He dropped it at the dealer and they replaced the EGR cooler & the pump. But the noise is the same and the smoke is still there - same as when he was driving. Under load the noise goes away, when he takes his foot off the pedal & coasts or idles it comes back.
He put around 200 miles on it like this - I don't see any rod flopping around for that long without seizing the motor. What else could it be?
The dealer hasn't let it go yet, they want to get more money out of him first!
The knock is steady, like you heard but the smoke is just like yours and when I just spoke to him he said the exhaust smells like fuel, not sweet like anti-freeze.
When you rev it up to about 2-2.5k does the knock sound better? You have a cracked/broken nozzle. I will bet on it. And if I am correct and he is driving it, he WILL melt a piston if he hasnt already. Unscrew the oil fill cap at idle. Got pressure coming out??? Puff, puff, pufff,???? Thats bad.

He said when he took off the oil fill cap there wasn't any puffing - just revving the engine only made the knock go faster - it didn't go away. It did go away under load when he was on the highway the other night however.
