Only surefire way I know of is to dissasemble the tailgate, clean out any rust and crap laying around inside, and coat it with a rust proofer, and Herculiner (or other kind of permanent coating that is waterproof). These tailgates are prone to crapping out exactly the way you described.

It also helps to replace the weatherstrip on the tailgate window every few years. See if yours is all dried out and cracked - if so, then water is getting past it into the tailgate.

Here's what i did to my 89 EB 351W. I removed the tailgate and grinded out all the rust. I sanded and wet sanded it, primed it and rust proofed the inside. Then i welded a rust proof stainless steel piece along the bottom of the tailgate and painted it. Never looked so good and rust free.
