Been 2 years now with an occasional sluggish acceleration after starting up cold. 2005 has difficulty accelerating and shifting and also smokes during 1st and 2nd gear while getting up to speed(55mph). Have had new high pressure oil pump, injectors scanned, new wiring harness to injectors, changed fuel filters and all updated programs. May go months without problem and then it returns. Dealer has no idea at this point. 200,000 miles on truck. Mileage has also dropped over past year and averages 12.9 but used to be 14.7

huh. have you done an injector flush? normally when i do mine i get my power back and also gain mileage. you could also try some stuff called Rev-X. its good stuff and cleans the oil side of the injectors. i would start there first since you have done all the filters and what not
Sounds like an air problem if it's smoking. Are still on the original turbo? Have you payed attention to your boost gauge? Is the problem seasonal, or comes up completely at random?

FYI: It doesn't clean anything, it just fills in the microscopic gaps the create a high coefficient of static friction/friction (think course sandpaper vs. fine sandpaper). It's also mostly filler (~75%) and the same results can be yielded for far less $$.
When I initially put my truck back together after a EGR bypass/cooler/etc. I forgot the MAP hose and as a result my truck has zero low end acceleration (no smoke or anything just couldn't build pressure due to the open hole in the intake manifold) so you might check your MAP hose and make sure it's attached and/or not cracked/plugged. Mine was pretty gross so I just ran a new stretch of hose to the sensor.
