I was talking to rickatic this afternoon and he said there were less than 1% of the trucks delivered have had mechanical problems. On my 3 other PSD's i never purchased them and honestly never had a situation where I wish i did.
I will consider it on this one since it is a new engine and drive train but it could be a waste of money.
I guess I am just thinking of the 04. Had to do the ball joints at about 90k and the egr at 94k. The injectors still scare me. That's one of my reasons for upgrading...........besides the fact I want one.
On our 2 current cars it has paid for itself on one and is 1/2 way there on the other. I know everyones mileage varies.......
I didn't buy at order but will before the factory 3y/36k warranty expires.
Probably within the next year (mileage) or sooner if I get word that prices will increase.
These new trucks are so dang complicated, even a minor failure could be expensive.

Now that's a good question.
When I purchased in May '10, my dealer said yes but that's because Ford was increasing the price of the ESP nationwide in June or July of '10.
I could have saved some money at that time but it wasn't part of my truck budget at the time.
Hopefully someone here will know because I certainly wouldn't depend on the finance group at the dealer to be 100% believable if you ask them that.
They can easily increase the price later and it's simply profit.
Not that profit is a bad thing (we all depend on it) but I've seen some really high ESP prices.
