Here's a picture of the one I built for my old truck. It was 2.5" tubes and 1/4" plate. I used the stock holes and one of the front hitch bolts to secure it, so it had 3 bolts per side.
I used a ramsey winch plate and mount it too my 4" tube bumber by making brackets that look like muffler clamps. Took some 3" by 6" square channel and cutting and grinding to fit then welding it on. (no u-bolts) This has held up real good. RKD.
