as for the transmission I'd get the Transmission serviced probly about 80 dollars thats what it'd be here in Houston,Tx and they'll completely drain the transmission both the pan and torque converter aswell as replace the Filter I BET that resolves the problem by it'self. with it being an E4OD it could also possibly be a sensor or electrical connection to the coil packs. BUT get it up to normal running temperature and park on a level surface throw it in park and Check your transmission fluid for any Metalic glitter or debree and also level ofcourse but more of interest is the health of the fluid is it burnt is it discolored or does it still look new???............barring that I've heard these transmissions can be had at a junk yard for about 350-500 depending on where you go Hope that helps

I have probably the same truck and If you have an E4OD/300-6 combo with 3.55 gearing, you can achieve 17-23MPG depending on constant highway/etc. conditions. I know I got at least 23MPG or better driving for about 1,000 miles. 98% was strict highway driving without a stop sign or red light to be had as far as the eye could see.
Right now driving about 34 miles a day back and forth to work, I'm getting about 17-18mpg steady.
