Answer
First make sure the service brakes are adjusted properly
Adjustment will either be at the junction of the 3 cables under the vehicle or at the pedal itself
Set pedal at 3 clicks and the wheels should be locked at that point ...

I would guess if your e brake is not holding, you probably need new e brake pads.. Remove rear rotors to replace pads if needed if the adjustment mentioned above does not cure the problem..
There is a star adjuster that you can use a flat blade screwdriver to turn, kind of like the adjusters used for drum brakes in the 'old days'. The longer you make the assembly, the closer the shoes are put towards the inner part of the rotor. Check to see if your wheels seals have leaked grease onto the shoes also. If you adjust it too much, you will break the cable...
There is an 'access' rubber gromett on the bottom of the dust shield area. Pop the gromett out and use a flat blade to turn the star wheel adjuster. If you want pics let me know!
Not completely true. My wife's van has drums on the back and you just simply put them in loose, start the van and start pumping, they will automatically adjust.
