Yes it sounds like an exhaust manifold problem. Usually the studs that hold the manifold to the head break off and need to be replaced.

I have had to do this type of repair to a Nissan Pathfinder that I owned. Let me tell you, it is not easy! I couldn't imagine doing it in the engine compartment on an Expy. The procedure I used was as follows: First go to the dealership and get replacement manifold bolts. Then get a set of easy-outs and a good drill bit. You will have to drill a pilot hole in the center of the exhaust manifold bolt that is broken off. Then you will have to screw the easy-out into the hole that you just drilled. Then with a small wrench you can remove the piece of bolt that is in the head. Put some anti-seize on your new bolts, thread them in by hand, and torque to specifications. If the bolts simply fell out, consider yourself lucky! I wasn't that lucky. It took me about 4 hrs to do 3 bolts! good luck

I know this isn't much help, but I'd imagine that this one of those things that, no matter the cost, is such a PITA that you have to go to the dealer.

Good luck, and let us know what happens!
