Egr cooler failure causes?

Egr cooler failure causes?
What would cause egr failure? Mine is being replaced again after 15,000 miles, along with egr valve, oil cooler and new turbo. Current milage is 70,000 miles. 05' 6.0 4x4 F250. With these parts being replaced are there other areas I should keep an eye on that could have been affected?

Usually crap in the cooling system plugs the oil cooler and that in turn overheats the EGR cooler causing it to fail. That is why it is recommended to change them both out. Now would be an excellant time to install a coolant filter.
