When in overdrive engine misfires or hesitates at 45 mph when under a load as going up a steep hill. Replace EGR and checked the dpfe sensor hoses and they aren't plugged. Any other ideas?
You are not giving me enough information. does the truck downshift (Assuming its auto), does the o/d light flash, is the check engine light on steady or flashing, what codes have you found, does it happen consistently or intermittent, do you have your foot on the floor or are you easing into it, does weather or temperature have an effect? I can help you if you can give me some basic info.
What you are describing sounds like torque converter shutter. Try changing the trans fluid. I bet the P0401 is unrelated. Did you clean out the ports in the throttle body?

Happens consistantly. Easing on the accelerator to maintain speed. Truck does NOT downshift as this occurs. The check engine light is on steady. The only code to show is the 0401. Weather or temp seems to have no effect. Replaced the EGR valve position sensor and the code has not reappeared but the problem persists.

That's because you had two separate problems and only addressed one of them.
I had the same problem at one point and it turned out to be cracked porcelen on one of the new plugs I put in the truck. My miss didn't throw a ck engine light. Sounds like it is time for a good tune up!!
My truck is doing the same thing. I changed the plugs a few months or so ago and fixed the problem. Last week it started doing it again. I changed the plugs again. Yesterday the ck engine light started flashing so i scanned it and got the 401 code again. In the boot of spark plug wire #4 there is some light grey lines where it looks like it could be arcing out. Once i change these ill let you know how it runs, this could be your problem as well.

Changing plugs won't have any affect on a P0401. You either have clogged EGR ports in the TB or your DPFE is bad.
