Engine idle problem when hot

My truck does idle weird when at normal temp ! both gauge Oil temp, water temps at 180. When I am at a stop sign it just mist to almost die. when it does that there is a small puff of black smoke coming out the pipe.

EGR deleted.

I suspect Fuel Filter again, they were change summer 2008.

Even when I drive steady lets say 60mph I can feel that something is not right. Steady pedal and it accelerate by himself a bit and then come down.

Any clue?

preparing to change filters again.

How many miles did you go since the 2008 filter change. You MUST change it at 15k intervals MAX! - Preferably 10k. Otherwise you risk injector repair bills.

Not knowing the build date, it could also be an STC or HPOP problem since it is only after the engine is warm.
