ok here is the story. i have a 86 f350 dually with a 6.9 in it. Engine now has about 60k since the rebuild. i have put a set of injectors in it last year. glow plugs 1.5 yrs ago. and today i put a fuel line return kit on it with all new caps, lines and gaskets. the inj pump doesnt look that old , and the injectors were factory one not cheap ones. any idea's?
I've had this happen, an injector gets loose from the head as I loosened the nut for its injection line - loosen all the line nuts and watch closely if an injector will try to twist in the head as you do so. I just tighten the offending injector back, then tighten the line nut - has happened a few times so far, sometimes by itself, so now that's the first thing I check when I get a funky idle or weird smoke out the stack.

when i did the fuel return kit today everything was tight and is even tighter now
So you double-checked on the injectors being tight in the heads? And before you do that you did loosen the lines, the tighten them back down when you were done? Well then, are you sure all the air is purged from the lines? How does the engine idle, does it shake like tis misfiring? (look at the washer tank for easy indicator for excessive engine vibrations)
I would double check the whole thing, when I finished my headgasket job I first found that one my return line caps was not seated fully, then, after getting bubbles and fuel at the line nuts, I thought I had the whole thing settled. After it ran for a little while I noticed some odd liquid (Fuel) bubbling on the pass front injector at the head, I had already tightened the injectors down to torque specs so it couldn't be that, could it? It was, I tightened the injector down a little more and the bubbles and the white smoke went away. Got rid of the air intrusion and got rid of the smoke.

Told ya it was an injector loos in the head!!! At least now I ain't the only one who has experienced this, lol. By the way for my truck the injector torque specs are just a recommendation, unless I lean on the wrench good them things always work themselves loose.
I put a new set of injectors in mine. Had one injector do the same thing. Back Driver side. So half inch drive ratchet and a 2ft long pipe later. It didn't leak any more and did miss either.
I work on mine with a 1" open-end wrenche, thing is over a foot long and barely even fits on the passenger-side injectors (darn evaporator core box is in the way), but I can tighten them while on the road without removing injection or return lines.
