Make it. You can easilly make this linkage from some regular old tubing purchased at your local home improvement store.
I have made these before, and they are not all that bad to reproduce.
They are bent to clear some of the components, but the mild steel tube can be bent easilly. Fill the tube with sand to prevent kinks, and bend it with a vice. Keep the bends big and not too tight, and it will look proffesional. Use your existing linkage to duplicate the ends. These factory ends are pressed and you can duplicate this is a vice. Install a pivot also available at the hardware store and you will be functional. This can be done without welding, but if you can weld, then you can really get creative with different style ends.
Should be pretty easy, and not as difficult as waiting to locate a difficult to find part.

Do what 350 said. I was having problems with my transfer case and it turned up being the linkage. Sounds like you have a 203 case like mine. I don't need low range so I eliminated my old factory linkage and made a new bracket on the tranny and a new lever to shift b/w 2h and 4h(t-case has conversion). I can still use low range if I want but I have to crawl under to shift. Someday I will make a double lever setup. Thanks to 350's advice mine works excellent now!!
