Could be any number of things. The suggestion would be to pull codes and see if there is anything showing up there that can lead you in the right direction.
You might have a look at the condition of the throttle body. Crud on the outside and inside could mean a dirty IAC (Idle Air Control valve and solenoid) assembly which can be cleaned carefully. If you attempt to clean these parts, make sure to keep electrical components dry and that any chemicals you may use are safe for your catalytic converter (if you still have one) and safe for sealed or coated throttle bodies.

have you checked out the electrical side lately rotor cap and spark plugs? sometimes they just sneak up on you and you really don't notice the overall lack of performance untill they won't idle. Always told keep it simple we usually get 300,000 miles on our rigs without much major work, just simple maintince
