If you were serious about one I think a couple micro switches mounted on top of the tranny could be used to disable the starter if the shifter is in a gear.
The remote starters are basically PCM controlled. The are simple logic controlers that take action based on input.
In the case of a diesel, you press the start button, and the glow plugs start,
When they are done, the PCM signals the starter to crank the engine,
Based on the tachometer signal, the starter is shut off once the engine fires,
If the engine does not fire after a preprogrammed number of seconds, the system shuts down.
An antitheft system usually comes standard with them.
Depending on what options you get with your remote starter, there can be a two way communication between your hand held controller an the PCM in the vehicle. So you will know right away when the engine has actually started running, or if it failed to start.
You could rig up a lot of little tricky theft deterency systems like a switch that only allows the truck to run (without the key) so the steering lock is still applied and they can't put it into gear in the case of an Automatic, all you would have to do is have a switch that applies 12v the the run position of the ignition ciruit and accessories so that you could have the heater/defrost on. You start your truck with the key as normal, flip the switch take your keys out and then go back in the house. Or for a manual it would just lock the steering wheel and you could also put another switch that would kill the engine if the clutch was pushed in when the key wasn't in the ignition. This would be really easy to rig up and wouldn't require much more than a toggle switch in the case of an automatic and for a manual possibly a micro switch or use the switch that is already on the clutch, or even use the Parking brake switch to kill the engine if someone tries to release the parking brake while the truck is running without the key in the ignition.
Nah just forget about all this fancy stuff and just put a couple loaded shotguns, one pointed at each door and tie the triggers to the door latches.
