rearend identification

rearend identification
I was wondering if any of y'all can help me. I wrote down what I think are the numbers/letters of the tag on the rearend. the tag is on the passenger side of the rearend bolted to one of the housing bolts. is this the right tag?
anyways here are the #s I have: (remember this is what it looks like to be, 29 year old truck).
