Different set of issues in play for that problem Campinnut.
Loss of forward is caused by several different possible issues.
For example, if you can put the truck into manual first gear, and it takes off normally? That would point right to the one way roller clutch being bad. That same part is also used for reverse---so it's not that likely to be the cause.
Bad bands/clutch pack....pressure issue...gummed up valve body..etc..

I decided to have it rebuilt. I called several transmission places in my area and got a quote of 1,300 for rebuilding mine. So i figured it was a good price.
That's about par for the course for a rebuild. Just be sure they install and use a new/rebuilt torque converter. I've seen some shops just flush out the old one--
Main problem with re-using the old one? All that sludge that gummed up the system in the first place, gets pumped around the TC. And it does have internal parts that do wear out. In general it might be a good idea to replace it at the same time it's rebuilt. They normally run about $85-$100. It's good insurance, plus the input seal at the pump? Normally cuts into the flange of the torque converter over time. They do make "Sleeves" that can solve that problem, but why bother taking a chance?
