the best 40" tires i had on my old truck were Pro Comp X Terrains. they were 40x13.5xR17. they looked sick, weren't super noisy, and did i say looked sick! i actually got better milage when i switched to these tires from the other Mickey Thompson's i had but those ones were 15.5" wide so that could be.
i also really liked the 39" BFG Baja Projects but those were out of my budget.

If you have 99-08 with serious cash the One Up Offroad setup with coilovers in the front or the front & rear is the way to go

I never recommend a body lift, cut out flares would be the best route if you dont want to lift it any higher.

With cutouts you only need about a 6" lift to clear 40s, maybe not even that much.

I'm running 6" Skyjacker and 3" body lift... the 39.5 IROKS I have rub on the leafs as well as the front bumper as well as the rear inner fender.

Proper offset would fix some of these issues, I've already removed material from the bumper... but you're going to want some offset as well as some trimming. They are going to stick out some if you don't want them to rub inside when you turn,.

On road I didn't rub with 40/15.50R20 Toyos. But I modified the corners of my front bumper when I frenched the lights in, so that helps a lot.

Only rubbing was on the leaf springs. Back on 38/15.50R20s now for more clearance.

For no rubbing with cut-outs you need 8" lift with 12" wide wheels and either trim the stock bumper or your ORT bumper will work.

or why not just go with a high enough lift to clear them without trimming..10in lift should suffice
