If there are enough threads exposed behind the nut I would get a thin jam nut to put on behind it. Any I dea why it is coming loose? What are the symptoms when it is loose, just curious?

You may have overtightened it at some point and stripped it. Very easy to do. You could find another nut of the same size and use it. Maybe a drop of loctite. I don't think you have room for a lock nut. Main thing is do not overtighen it. finger tight plus about a 1/4 turn at most.

You may be able to shorten the spacer & double up on the nut....
