Injector driver mod problems
A lot of the IDM problems we see that are associated with a bad running engine come from either a loose connector of chaffed wires under the valve cover. We call this UVCH (Under valve cover harness).
You can check the wires with an ohm meter at the 9 pin connector that plugs into the valve cover. The wires in the connector are laid out as follows
G G I I C I I G G
G=Glow Plug
I=Injector +
C=Injector -
Ohm Glow plugs between "G" and Battery Negative post. Should be between 0.1 and 2.0 Ohms
Ohm Injectors Between "C" and "I" should be < 5 ohms
Here's a link with pictures that shows you what some of the problems are

Had the valve covers off one day for a different reason and upon the initial start up after the completion of the repairs the truck had a dead miss with only P1316 code showing.
Turns out that I accidentally hooked the retainer clip on #1 injector and it came unplugged as I was re-installing the valve cover on the engine.
