I don't have one on my truck, but I've driven trucks that had one installed. Man, what a difference on the downhills. Saves the truck brakes, too. Depending on the grade, you may not even have to touch the brakes going downhill.
An exhaust brake can seem expensive up front, but depending on how much towing you do and the weight you're towing, it may be cost effective in the long run by not having to replace your brakes as often.
