No plug blow out issues on the 5.4L 3V....but you'll read lots of nice stories about plugs being broken when attempting to change spark plugs. Do a search you'll see. In summary, changing the plugs long before the 100k mile recommended interval seems to help (~50k or less)...follow the TSB (technical service bulletin). I changed mine at 41k with no issues.
There has been no head design changes from 2004 to date....except for some minor cam phasor changes between 2004 and 2005.

After being told to wait until I absolutely have to by the Ford dealer, I was told by the same person that it's probably a good idea to remove the plugs every 30k and apply anti seize on them. This is after I paid a chunk of change to have 6 broken plugs extracted. Just an FYI-not sure if it will actually solve the sticking issues.
