The D5 means its 1975. As for the model of the carb (i assume you are looking to buy a kit for it) it should be stamped somewhere on the carb. Look on the baseplate or airhorn. Those are the numbers that you will be able to cross reference at an autoparts store. I wouldnt trust the tag. It could be for the engine if someone placed it on the wrong place.
your carb is a ford 2/b originally off of a 75 p.u. with a 360-390. be sure to keep that tag with the carb, if you need a kit or any parts for it you will need it. D5TE ATA is what the parts house will need.
