Hey BTH could it be a wire chaffing issue??? I had an 03 that had a wire chaffing issue under the dash along the steering column and these symptoms sound very familiar to mine, it also was a real bugger to find....
Thanks for continuing to think about this one. It's so sporadic and doesn't really seem to coincide with anything except a warm engine. I filled up (USLD) at the same station twice, used PS white bottle, and it did it on both fill ups. Half filled up (LSD...forced to) at a station this last time and added half bottle of PS white bottle and it did it.
When it does it, the darn thing stumbles like a Harley exhaust system sounds only not as extreme as a Harley. One of the first times I took it in to the SD the counter person joked that it sounded like I had a (performance?) cam in it.
ok well try this, unplug your icp sensor and drive for a while to see what happens. your light will come on, and it may be hard to start due to the sensor unpluged, but it wont hurt a thing because it it will go to default. i know exactly what sound you are talking about now and it is caused by icp pressure change whether wanted or not. becareful when you remove the connector because the white lock my stay in the sensor. due this for untill you think it is long enough, and let me know.

A couple questions:
1) By doing the above, it will not give my SD (or Ford) any reason to say 'that was a no-no' since I'm assuming a code will be thrown?
2) By driving for 'a while' do you mean drive as I normally would until I think an event would have occured...like a week?
3) Where is this leading (symptom/diagnosis)? Bad icp?
I'm glad the description of the sound/symptom may be helping. I'll re-clarify that is does stumble like an Harley's exhaust sounds, but not nearly as extreme. It's more like a normal running engine with a stumble (or two/three) every 15-30sec.
your dealer should not have a issue with this because this is process of elimination. if it still does it with the sensor unpluged them it probably something else, like a cam sensor or crank senor. also try to notice if either the glow plug light or the rpms drop out when ever this happens.

No glow plug light.
No significant rpm drop out. Just a slight variation on the bigger stumbles. I tried to video tape it (RPM gauge), but it was low light.

I thought I'd check back in to let you all know I'm still having the issue. Normally only does it after sustained highway run. Then at idle at end of exit ramp it starts to surge/miss.
The kicker is today it finally did it 1/4 mile from the dealer. I called ahead and asked the diesel tech to meet me, but...I kid you not...it stopped doing it when I pulled onto the lot.
Tech took a ride with me and it didn't do it again. At least we got to chat a bit and he hooked it up to the computer...no codes again.
I never unhooked the icp and the tech was glad b/c he didn't want the code to lead him down a potentially wrong path. It's going in for oil, inspection, and brakes (Powerslot cyro and Hawks all around...thanks DPPI) next week, so hopefully we'll find out more.
Anyone want to hear the noise on video??? Listen closely for the surging, not the general running of the engine, but the surge. Hopefully the quality is good enough.

In the first video, if you listen closely you will hear my step-daughter say "What's he doing" and immediately following you will hear the engine surge/miss.
In the second video, you have to listen more closely because the noise is not as filtered as inside the cab, but you will hear me say "put it back in drive" and then you can hear it.
