KOEO - Key On Engine Off - a diagnostic that is run while the engine is off.
You need a more comprehensive diagnostic utility such as Autoenginuity to perform things like KOEO diagnostics and injector buzz tests, which will retrieve the IDM codes.
If you don't have the tools or equipment, it's best to find a shop that can do it. From your original post, it sounds like your diesel mechanic doesn't have any idea what the problem is. He is swapping parts out in an attempt to fix the problem, which only adds up on your bill. Take it somewhere else.
Your problem sounds similar to what I am just now staring to experience, ie code 603, hard to start, surging or rompimg when first starting, no throttle response until it warms up. I'll be watching for a fix also!
