I'm running a NV4500 in my jeep with an atlas transfer case and it's killer. There are a few different versions out there from both GM and Dodge, the old ones had a really low granny. You'll need to do some work to match it up to your bell housing and transfer case, probably need to get a different bell housing and/or adapter plate. You can fab one up if you're handy. You will also have to deal with a new clutch, linkage, etc. It's a pretty big job to get everything to work, a clutch slave might be the way to go. Or sell the NV4500 and go with a Ford 5speed, probably a lot easier. Auto to manual conversions can be done, they just take time, money and ingenuity...

The NV4500 had some serious 5th gear problems in the earlier models especially when towing. I think they got most of that resolved later though. There are some aftermarket companies selling kits that make these trannies almost bulletproof. I think you could probably buy all of the stuff to bolt in a ZF5 for what the NV4500 would cost.
