Loose Lug Nuts

Has anyone experienced lug nuts that won't stay tight no matter what you do?
I have replaced the rotor and lug nuts but they still won't stay tight.

I see loose lugnuts every time I drive on I95 -

OOPS - sorry -couldn't resist.

Anyway, where did you get the lug nuts? How did you torque them? Are they the correct nut - ie, right taper for the wheel, etc?
