My company has six 6.0 Super Duties. Removal of the HFCM cap was very difficult on three of them. It literally took 2 men and a 24" breaker bar to get the cap off. On the first one the threads appeared to be damaged on the plastic cap so I replaced it ($60.27). The second one was equally difficult to remove so it was replaced also. When I went to the "Dealer" to pick up the new cap I asked the Service Manager if they have had this problem, naturally his answer was NO.
The diesel mechanic was near by and offered that fuel additives can cause the plastic cap to "swell", making removal difficult. We use Power Service Diesel Fuel Supplement + Cetane Boost in the winter. The third cap was tuff to remove but threaded back in easily. The forth truck that was serviced, the cap was
"easy off-easy on".
Has anyone else experienced this problem or have an opinion on the fuel additive issue?

I call BS on swelling the cap. It's interesting that your fourth truck, which I assume has also used PS, had no problems. I would lean to over-torque when the cap was installed on your other trucks.
However, in about 2000 miles I'm going to find out on my truck. I've been using PS since the second tank full and I'm now approaching 15k miles.
They are hard to remove. I dont know why. I was able to get mine off with a ratchet, but i had to use the ratche at 1/2" turns until it got to the very last thread. There is no loosening it and unscrewing it with your hand.
