a highboy is called that due to the frame design. the F250 4 X 4 frame was a long narrow ladder frame that necessitated a high stance for all the components to clear each other and work the way they were designed to (tires don't hit the frame on turns etc...). hawkrod
Highboy is just a term, and I think how much lift it took to get your butt up on the seat had something to do with it, too.

1995 Ford F-250 4X4, Powerstroke Diesel. Supercab longbox. 70 hp chip, isspro pyrometer and boost guages. Other than that its stock. Plans: 4in exhaust, propane injection, cd player, and possible intercooler.
