You can get a rebuild kit from Classic Performance, part number 5760SOK for $260. I bought one, but then found that a salvaged steering box I already had was in great shape, so I can't really say how hard it is to install.
For instructions it includes a copy of an article in Classic Trucks July 1999 which shows the procedure that CP does when rebuilding a box. There's some grinding and honing to do, and a pin needs a weld to hold it in place. http://www.classicperform.com/4856fordtrk.htm
