That's a Ranchhead Headache Rack that has been painted gray. Got one on my 2005 F250 in the original gloss black.

I had a Ranchhand unit like that on my 99, but Ranchhand doesn't make the integral headache rach/bed rail protectors anymore. (Although you can still get the 3-piece system.) Additionally to those mentioned, SouthWest Fabricators makes a couple of model of which the 305 Headache/rail protector rack is very similar to the picture.
