Does this mean that erasing the memory codes with my code reader does not clear the memory completely?

There is an occasional error code 42 that seems to appear from time to time. The O2 sensor has been replaced twice recently. When the code is present, the engine seems to stumble when the transmission is first engaged (automatic), slowly recovering by surging until it finally settles down. There is also hesitation when accelerating, under load. I've replaced the ignition wires, distributor cap/rotor, spark plugs. The engine has good compression and, when running correctly, very normal vacuum. Spark plugs come out black and covered with soot, indicating a rich condition.

As I interpret the running code (42), it means the signal to the ECA from the O2 sensor is always rich. What else could be causing this? There are no KOEO stored codes other than 11. I've replaced the fuel pump, pressure regulator, MAP sensor, water temp sensor, TPS, EGR valve, TFI module, coil and air temp sensor. The only remaining items, yet to be replaced, are the ECA and fuel injectors. Oh, I almost forgot! ... The entire truck!
