The other day I got home from town with my dad's 1994 f-350 and the lock out for the 4x4 was gone. O.K there are five or six hexagon head bolts on them could it just fall off? I don't think it could fall off with out help.
You lost the entire lockout hub? Sounds like a retaining ring was not installed...the six screws only hold the cap to the body of the lockout hub. The body is retained by a large C-clip to the brake rotor/wheel hub assembly and in most cases a smaller c clip retains the axle shaft also, this smaller clip may not be required on your live axle 350....
I find it hard that the entire hub fell out. On your truck, there is a small c clip that holds the inner part of the hub in. The outer part of the hub is held in my a retaining ring in the actual hub flange itself. The 5 screw you are talking about only hold the cap on. This is all as stated before.
It sounds like someone did some front end work recently and left out a few things.
