just remover the caliper and the rotor should slide off. by the way inconsistant torque on the lugs will cause this. never use a impact to tighten lugs on a disk brake setup. only use a torque wrench.
So no need to dis-assemble the front hub to get the rotor off the truck?

Take it in to your dealer
A warpped rotor can be caused a sticking caliper(heatfrom not retracting) and over tightening of lugs.
Also by a pads slider shoe not being lubricated properly
