Replace clutch or master cylinder?

I have a 1992 F150, 300-6 cylinder (124,000 miles), 5-sp transmission. While driving home from work I wasn't able to downshift from overdrive into any of my gears. I also had to put my foot behind the clutch pedal and pull to get it off the floor. I finally got it into first and drove it home. Sitting in my driveway, with the clutch pedal either engaged or disengaged, I could only move the stick into first or reverse and not into any other gears. When I start the truck and let off the clutch pedal with the stick in neutral, it grinds. Does this sound like the entire clutch assembly or just the master cylinder needs to be replaced? Is this a transmission problem? Thanks.

Check the fluid level in the clutch master cylinder.
If that is ok it could be the master cylinder, slave cylinder or clutch.

Thanks for the info. The fluid level appears fine. Even if it is only the master or slave cylinders, do you think replacing the entire clutch assembly at 124,000 miles is a good idea?
