My 98 Explorer 4wd XLT has 125K and I have metal in the PS reservoir, and when I turn the steering wheel while driving I get a loud clunk or grind and then whining. Think I need a Rack and Pinion and Power Steering Pump. I have flushed it once before but the metal returned.

Is there anything I can do short of replacement of these items?
How long is it safe to drive in the present condition?
Any precautions I should take with the new systesm to avoid reocurrence of this problem?
