My V-10 just turned 15,000 miles (second engine), see my post smoke on start up. The 1st engine, knocked, puffed a cloud of smoke on a cold start up, had sever pre-ignition whenever I worked it hard, had exhaust flutter like I've never heard and raced at 1500 RPM on a cold start. Ford replaced it at 3000 miles, It's a long story and I'm not going to go into it now. The second engine, Ford called it a quality renewal engine. Has never used any oil between oil changes, won't pre-ignite no matter what I burn for fuel, never has noise on a cold start unless I get an oil filter with an anti-drain back valve that leaks, then I replace it and the clatter doesn't happen until I get another leaking filter valve. Has no exhaust flutter at all, and starts on a cold start at just over 1000 RPM. The engine in my truck now, runs like a V-10 should, quiet, strong, smooth. But it was one hell of a ride to get here, as much as I like the excursion now, I don't think I will buy another ford. They really treated me wrong, if it weren't for the dealer, I'de be in a chevy now. Good luck, get your truck fixed.
I have a 00 with the 5.4. At 8K miles last year, I reported an engine ping/knock to my dealer. It was at cold start while reving the engine at about 1500 or so. During the summer it goes away. In the winter it's bad. It usually goes away after a mile or so of driving, but it's gotten a lot worse. It now sounds like a loud clattering.
After a year of visits, and exchanging the Y pipe and using clamps on the exhaust, they finally admit that it's the engine, so they are going to replace it. BUT, Ford won't give me a new one because I have over 12K miles on it and it's been to long, even though I reported it at 8K miles. I have 23K miles now. They are giving me a Ford remanufactured engine to the same specs as my old engine. They are also giving me a premium 75K bumper to bumper warrantee on my truck to go along with it for the inconveinience and because I complained.
To be honest, the rebuilt engine may be better than a new one off the assembly line. It just seems that they'd have to pay a little more attention to detail on a rebuild, than on an assembly line.
I personally think they knew it was bad all along, but tried to make me go away. Two of my friends have listened to it and without me saying anything, they said it was the lower end of my engine.
If you want a brand new engine, make sure you get it done soon or they will give you a rebuild. I may have had legal recourse, but I just don't want to deal with it. The Warantee is a good deal also. I get my new engine on Monday.
Just got an answer from Ford about my engine knock and I'm PISSED. They told me that it is an operating characteristic of the motor and they are not going to replace it but did offer me an extended warranty. This sucks I can't believe this is their solution to the problem. Also did check my doors and they do rub on the driver side guess I bought a lemon.
One way I found to make it happen really bad is this:
After starting cold, drop it into 1st gear, and drive at about 1500 - 2000 rpm. This causes my engine to clatter something awful, and doesn't clear up until the engine is warmed up and the oil is flowing, usually 5 or 10 minutes, or the first mile or so.
If you don't hear anything, then maybe it's not so bad. If you do hear it, and it's worse than the sound it makes when idle, let the dealer hear it. I'd whine and complain for a while before accepting it. It took me a year to get something done. Like I said, it got worse with time. Also, it got worse after they changed my exhaust Y pipe. I think the added pressure from the smaller pipe made it worse. I think that pipe is on the 2001 models.
just got back from 2 other local dealer's and I am going to get a second opinion from them. Also talked to a friend of mine who is a Ford Tech. and he started laughing when I told him what the dealer said. He thinks the dealer is behind this BULL he's going to call Ford and tell them that he has the truck in front of him and it does make the noise to see what they say first hand.
There is a TSB out about roller finger followers being dislodged from the valves. Maybe that's your problem.

Rough idle/miss after start up during cold ambient temps less than 10 degrees F
Issue:
some vehicles equipped with 4.6 5.4 or 6.8 L engines may exhibit a rough idle/miss after start-up in cold temperatures. This may be caused by a roller finger follower being dislodged from the valve. This may result in damage to the camshaft,, roller finger follower, valve, retainer, etc.
dfs hit the nail right on the head. My fist V10 would clatter on start-up and I could get it to clatter by slightly lifting the accelerator as the rps's build just before the auto shift. Ford installed a FQR engine. I have 31k on the rebuilt engine. All was well for a while. Now, it's quiet at start-up but clatters pretty good as you drive. Been doing this for a while now. Once it's warmed up it quits.
I OWN A 2001 250SD CC SB 5.4 AND IN THE COLD MORNINGS IT MAKES A LIFTER NOISE BUT ITS A OHC. IT ONLY LASTS FOR A FEW SECONDS BUT IT BUGS ME .MY LAST TWO TRUCKS ,WHICH WERE DODGES,MADE THE SAME NOISE BUT I EXPECTED THAT FROM THEM. I USE ,IN MY OPINION, THE BEST OIL + FILTER COMBO AVAILABLE. BALDWIN FILTERS AND AMSOIL SERIES 2000 0W-30. THESE FILTERS HAVE SPRING ACTIVATED ANTI-DRAINBACK AND BYPASS VALVES, WHICH IS BETTER THAN SILICONE OR RUBBER. WITH ALL THIS SAID MY TRUCK STILL MAKES NOISE OTHER THAN THAT IT RUNS EXCELLENT.
My V10 used to have a "valve train rattle" at start up when new. I replaced the stock Motorcraft filter @ 1500 miles with a Purolator. That was the end of that noise.
Now, as far as all the complaints of "bad" engines and the "dealer run-a-round" scenario, you fellas may me for this, but spending $150.00 for a well written letter from an aggressive attorney should get you a faster response to your issues than spending endless hours and trips to your dealer. Money talks, and if they think they will have to not only buy you a new truck, put pay "reasonable" attorneys fees in a lawsuit (threat only), it should bring them to a resolution faster. I know this works as I'm married to an attorney and she gets results...her phone # is......

My FIRST F250 SD 4x4 PSD had the cam shaft come apart in the first 80 miles on the odometer. The morning after I bought the thing, the dealer found it back in the service area (after being towed in of course). I told them that it had sounded like the engine was coming apart. Later that day they told me what had happened to the engine and that they would be happy to fix it promptly. I told them that would be fine but I sure wasn't going to take it back. Told them I would never trust the truck again. I managed to convince the dealer to find me a another truck and we re-did the paperwork. It took some stern words (polite, of course) to make it happen but they did the right thing. I think they were shocked that a PSD could come apart like that. Everyone agreed they had never heard of such a thing.
