I don't think my van's ever had it, but it's definitely some sort of striker that goes here for the right cargo door. Can anyone help me figure out what it is, and better yet, if I can get a replacement? Thanks

I would agree, my '81 E150 had the upper read door striker there. It was a steel rod with two 90* bends with flattened ends to be mounted. I have no pic though, sorry.
Not to be critical, but knowing the local Ford dealer as I do the website may not be of much help for an earlier van like a '91. I was told that, at least on the local level Ford ceases to carry parts for vehicles more that 5 years earlier than the current year.
Try a new post with a subject line like "need help with body part number for 1991 E150." Someone with a parts book might oblige with a part number you can use to search eBay.
If your picture shows the location I'm thinking of, other vans have a striker. I assume there's a corresponding working latch on the door. Maybe the striker was removed because the latch won't release.
Check car-part.com for a compatible van in your area then visit the yard. I find that most yards aren't helpful over the phone for small parts that aren't itemized on car-part.com. We'll call you back means they won't :/
Our 2007 has a similar striker for the driver side rear door (no upper striker for the passenger side rear door) on a 3-3/16" screw base but the striker bar is angled to the passenger side. A welder should be able to angle it how you need it. Striker shown below between door switches.
